Community Health Needs Assessment

Every three years, Habersham Medical Center, along with other healthcare partners located in the Northeast Georgia region, work together to gather critical information about the overall health of our communities. Information is collected over a three-month period through multiple survey channels and is used to create a regional Community Health Needs Assessment. This assessment is a requirement by federal law and nonprofit hospitals must share the outcomes with the public as well as develop implementation plans to address the most pressing issues presented in the report.

This interactive dashboard was created by Northeast Georgia Medical Center to display the Community Health Needs Assessment by area. The community health data provides a look into health and social factors affecting communities and includes county and zip code health indicators compared to state benchmarks when available.
