|  | Habersham Medical Center's medication reconciliation program encourages people to always carry an up-to-date list of all the medications they are currently taking including over-the-counter, prescription and herbals. This will help minimize the possibility of medication errors and harmful drug interactions. For your convenience, a medication sheet or card is available here for your use. You are encouraged to print the medication card, complete it and put it in your wallet. And, don't forget to update the information as needed.
