|  | For inpatient admission and outpatient procedures, patients should sign-in at the main entrance information desk and complete the registration process in the private registration area located nearby.
For your convenience, you may register on-line by clicking here. If you choose to register on-line, please check-in at the main entrance information desk upon arrival.
Copies of your medical records or diagnostic images can be obtained in the registration area. Patients must show proof of identification to obtain medical records.
To complete the registration process in person, it is very important that patients bring the following items every time they register:
- Photo ID
- Current Insurance Card(s)
- Physician's Order (except for Emergency Department visits)
