Habersham Medical Center offers patients who are hospitalized with bright, comfortable rooms filled with natural light from oversize windows overlooking Northeast Georgia's ever-changing seasonal landscape, and the medical team has vast experience and training ranging from pediatrics to geriatrics.

Intensive Care involves two critical elements: a 24-hour-a-day advanced cardiac life support-certified staff of nurses and monitoring and life-support equipment that reflects state-of-the-art technology.

The experience of Habersham Medical Center’s ICU nurses averages more than 20 years. Our digital monitoring equipment is so accurate that it can recall the specific parameters of a patient's heartbeat that occurred 24-hours earlier. And from the ICU, nurses can monitor a patient's heart even though he or she is located in a patient room on another floor.

Case Management
Case Management at Habersham Medical Center is a collaborative, multidisciplinary process that assesses, plans, implements, coordinates, monitors and evaluates the options and services required to meet an individual’s healthcare needs after discharge. Effective communication and available resources are utilized to promote quality, cost-effective care.

Case Management begins on admission and is on-going through discharge. Our case managers coordinate the clinical goals with the patient’s goals and manage the financial and quality outcomes of the care provided.

Referrals for visiting nurse services, skilled rehabilitation, nursing facility placement, and third party payor clinical reviews are daily functions of the Case Management department.

Hospitalist Program
If you are admitted into the hospital, your primary care physician may refer you to the care of a Hospitalist physician. Or, if you don’t have a primary care physician on staff at Habersham Medical Center, your care will be provided by a Hospitalist. Hospitalists at Habersham Medical Center are board certified or board-eligible physicians who specialize in treating hospitalized patients. As with other specialists, they are an extension of your primary care physician.

Surgeons and other specialists may also ask a Hospitalist to participate in your care to help manage the medical issues you have chronically or develop acutely while they are treating you.

Some of the benefits of having a Hospitalist care for you include:

  • Hospitalists have expertise in treating illnesses in the hospital.
  • The Hospitalist is available in the hospital, a few steps from your room, all day, every day. At night, they are either in the hospital or a short distance away to provide quicker access to you and the hospital staff.
  • Often, patients’ hospital stays tend to be shorter because Hospitalists are available to discharge patients throughout the day instead of just in the early morning or late evening.
  • Because your primary care physician does not have to respond to hospital emergencies and questions, they can give you their attention in the office with fewer delays and interruptions.

Your Hospitalist will communicate with your primary care physician on admission to the hospital, if there are significant changes in your condition, at discharge and at any time at your request. Communication and transfer of information is very important to your ongoing medical care.

These are the four primary Hospitalists. They typically work several days and/or nights in a row then are off a few days, at which time they will give the next physician detailed information about your condition.

The Hospitalist program at Habersham Medical Center is one of the ways we try to provide you with the best medical care possible. Hospitalist services are contracted through Incompass Health/24ON Physicians.
