The entry fee is $125 per person or $500 per team and includes beverages, snacks and lunch. Fax or mail your entry form by Sept.10, 2010. Checks made payable to the HMC Foundation, Inc. can be mailed to the HMC Foundation at P.O. Box 37, Demorest, GA 30535. Fax number is (706) 754-7300.
Format:
18 holes, Scramble Format, and a 1 p.m. shotgun start. Please reserve early, number of teams is limited. Priority is given to sponsorship levels.

Proceeds from previous HealthLinks golf tournaments have purchased new exercise equipment for HMC Rehab Services, a Stereotactic Breast Biopsy System, 12 new patient beds for the med/surg unit, the rooftop Emergency Department helipad and a Cardiac Rehab Telemetry Unit.
The HCMC Foundation, Inc. is a 501(c)3 non-profit tax exempt organization formed in 2001 to develop relationships and financial resources to ensure HMC remains at the forefront of medical advances; 100% of all gifts, minus fund-raising expenses, benefit the program or department designated.
